Your name should be your name as you want it to appear on your Membership Certificate. Your birth date shall be – day, month, year (e.g., 15 January 1950). Do not abbreviate the city, county, or state. Your birth date must be supported by a birth certificate, or other evidence, showing your full date of birth and naming both parents.
Father and Mother
Identify your natural, not adoptive, father and mother, and the dates and places of their births, deaths, and marriage.
This procedure shall be followed for each succeeding generation, to and including that of your qualifying ancestor. Proof must clearly tie each generation into the preceding one. In each generation, give the names of the natural parents as part of the lineal descendant of the applicant. The terms “junior” or “senior” shall not be used on Page 2. Likewise, omit titles such as “Doctor,” “Reverend,” etc. An applicant shall give complete dates and places for the first three generations. The remaining generations shall be as complete as possible, and proof submitted for each date and place given. Where dates and places of birth, marriage, or death are unknown, leave these spaces blank. No application can be accepted with a large amount of data lacking for these generations. Exceptions may be made when it can conclusively be proven that the lineage is correct. An application cannot be accepted without one date, age, or place per person in each generation. Dates and places of birth and death must be given for the San Jacinto Ancestor. When exact dates cannot be given, approximate dates, such as those based upon dates of marriage, dates of wills, deeds, etc., shall be furnished. In all cases, it must be shown that the ancestor was living in the place where San Jacinto service was claimed, and was of sufficient age to have performed the service claimed for him if he was not on the Honor Roll.
The San Jacinto ancestor’s lineage is always the last generation in the line of descent. If known, give the place of burial of the San Jacinto ancestor, (name of cemetery, town, county, and state).
Proof of service at the Battle of San Jacinto
Most applicants’ ancestors will be listed in the Honor Roll of the Battle. If your San Jacinto ancestor is not listed there, a copy of the service record, or the Bounty or Donation Certificate, showing the said ancestor’s service in the battle of San Jacinto must accompany the application.
Genealogical proof
One copy of each piece of evidence must be submitted. Proof is required for all dates, places, and relationships given in the lineage. If a previous application paper has been used for reference, please furnish a copy making sure all documentation currently required is furnished. Be sure that the parentage of each person in the direct line is documented. The lineage form applicant to a common progenitor must be included. Examples of acceptable proof are:

Lineage papers from other organizations, family group sheets, and charts, are not acceptable proof.
